AMATEUR ATHLETIC WAIVER AND RELEASE OF LIABILITY
In consideration of being allowed to participate in any way in the TEMPLAR CLASSIC 3V3 SOCCER TOURNAMENT:
1. I agree that prior to participating, I will, or if I am the parent or guardian of a minor participant, will instruct such participant that he or she should inspect the facilities and equipment to be used, and if I believe anything is unsafe, I will immediately advise my coach (if I am participating as an athlete) or supervisor of such condition(s) and refuse to participate.
 
2. I acknowledge and fully understand that I may be engaging in activities that involve risk of serious injury, permanent disability, or death, and severe social and economic losses which might result not only from my own actions, inactions or negligence by actions, inaction or negligence of others, the rules of play, or the conditions of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time.
 
3. I assume all the forgoing risks and accept personal responsibility for the damages following any such injury, permanent disability, or death.
 
4. Intending to be legally bound, I do hereby release, waive, discharge, and covenant not to sue MANTI HIGH SCHOOL, sponsor of the TEMPLAR CLASSIC 3V3 SOCCER TOURNAMENT, its affiliates, their respective administrators, officers, directors, agents, coaches, and other employees or volunteers of the organization, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and leasers of premises used to conduct the event, all of which are hereinafter referred to as “releasees,” from any and all liability to each of the undersigned, his or her heirs and next of kin for any claims, demands, losses, or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releasee or otherwise in connection with association or entry in and/or arising out of my travel to, participation in, and returning from competition. 
 
5. In the event that I sustain injury or illness while competing in the TEMPLAR CLASSIC 3V3 SOCCER TOURNAMENT, I hereby authorize any emergency first aid, medication, medical treatment, or surgery deemed necessary by licensed medical personnel. I also give my permission for attending medical personnel to execute on my behalf my permission forms or other necessary medical documents and to act in my behalf if I am not immediately able to do so.

2011 Winners













Manti's Lady Templar Soccer Team Hosts 2nd Annual 3 on 3 Soccer Tournament



In an attempt to promote soccer in Sanpete County the Manti Lady Templar Soccer team hosted the second annual Templar Classic 3v3 Soccer Tournament, on Saturday, May 14th. 29 teams ranging from middle through adults and travelling from as far north as Spanish Fork and as far south as Richfield competed in this event.



After a grueling day of soccer six teams were crowned victorious.

2010 Winners

Manti's Lady Templar Soccer Team Hosts 3 on 3 Soccer Tournament

  • Team Registration Form and Fee must be received by May 19th to guarantee your spot. Completed Team Registration Form
    and fee may be turned in to Manti High School, the Manti City Building, the Ephraim City Building, or mailed to Eleshia Steinfeldt P.O. Box 650068 Sterling, UT 84665.
Registration Deadline: No teams will be added after Wednesday, May 19th at 7:00 PM. Schedules will be posted on the Templar Classic blog (http://www.templarclassic3v3.blogspot.com/) and emailed to the designated coach/team representative on Thursday, May 20th.

TEAM CHECK IN (MANDATORY): A coach or team representative from every team must check-in at the Referee Tent/Check-in area on May 22nd at 8:00 a.m. Each representative must hand in completed Team Roster and Liability Waiver with all waivers signed by each team member if over 18 or by parent or guardian if team member is under 18, including those who may not be present for the first game. Any team that has not checked in at the Registration/Check-in area will forfeit ALL games until this task has been completed.
Team Size: 5 players maximum; one player must be designated as a coach. Middle School and High School divisions must have an adult representative. (No roster additions allowed after team check in.)
Divisions: Need a minimum of 4 teams to hold each division. Age determined by participant age on the day of the event. Younger players will be allowed to play up a division, but older players will not be allowed to play in the younger divisions.

Venue:
Ephraim City Baseball Complex (West Fields) - 350 N. 200 E

Rules: 3 on 3 soccer has many of its own rules. Please learn them well before the competition. Any rule not specified herein will be enforced according to the National High School Federation rule book.
Player Registration: All players must be registered on their respective team's roster. No roster changes after team check in. Players may play on only one team.
Number of Players: Maximum 5 players per team, 3 field players and 2 substitutes. A team must have a minimum of 2 field players. There are no goalkeepers in 3 on 3 soccer.
Substitutions: Substitutions may be made at any dead-ball situation, regardless of possession. Teams must get the referee's attention before substituting.
Uniforms: All players must wear shirts during play and each team must bring both a light and dark colored jersey. If both teams are wearing the same color the team listed first (Home) on the schedule must change.
Equipment: Teams must provide a game ball if called upon to do so. The team listed first (Home) on the schedule is responsible for checking out a game ball from the Referee Tent/Check-in area if neither team has one. All players must wear shin guards.
Field Dimensions: The playing field is 40 yards long by 30 yards wide.
Goal Box: The goal box, is an arch ten feet wide by six feet long directly in front of the goal. There is no ball contact allowed within the goal box, however, any player may pass through the goal box. If the ball comes to rest in the goal box, a goal kick is awarded regardless of who touched it last. Any part of the ball or player's body on the line is considered in the goal box and is an extension of such. If a defender touches the ball within the goal box, a goal is awarded to the offensive team. If an offensive player touches the ball within the goal box, a goal kick is awarded to the defensive team.
Game Duration: The game shall consist of two 12-minute halves separated by a two-minute halftime period (26 minutes) OR when one team reaches 12 goals, whichever comes first. No timeouts in 3 on 3. Games will be played every 45 minutes.
Goal Scoring: A goal may only be scored from a touch (offensive or defensive) within a team's offensive half of the field. The ball must be completely on the offensive half of the field, and cannot be touching the mid-line.
The team who scores is responsible to report the goal to the referee and turn the score card before play begins again at the kick off.
Playoff Overtime: Shall consist of a 3-minute golden goal overtime period with a coin toss to decide kickoff/direction. The first team to score in overtime is the winner. If no team scores in the 3-minute overtime, the winner shall be decided by a shootout. The three players from each team will enter a rotation of penalty kicks, alternating teams with each kick, with the higher scoring team winning after the first round. If the score remains tied after the first round of penalty kicks, the two remaining substitutes will alternate in a sudden death penalty kick format until one team scores unanswered. If a team has received a red card during the game and finishes with two players on the field, a remaining roster player, other than the red-carded player, may be chosen to kick in the rotation of penalty kicks. If the red-carded player is the last remaining roster player, one of the two field players may kick twice. The opposing coach has the option to pick the player from the bench/field of the team that is left down a man after time expires.
Offside's: There are no offside's in 3 on 3 soccer.
Slide Tackling:
Slide tackling is not permitted. Players can slide for a ball if there is no contact or intent to contact initiated. For example, a player can slide for a ball that is going out of bounds, if no other players are around, but a player may not slide and take or attempt to take the ball from another player in possession of the ball.

Five Yards: In all dead ball situations, including kickoffs, defending players must stand at least five yards away from the ball. If the defensive players' goal area is closer than five yards, the ball shall be placed five yards from the goal area in line with the place of penalty.
Kick-ins: The ball shall be kicked into play from the sideline instead of thrown in.
Indirect Kicks: All dead ball kicks are indirect excepting corner kicks and penalty kicks.
Goal Kicks: May be taken from any point on the end line.
Kick-off: Is an indirect kick and may be taken in any direction, including backwards.
Penalty Kicks: Shall be awarded if, in the referee's opinion, a scoring opportunity was nullified by an infraction. The infraction does not automatically result in a red card. It is a direct kick taken from the point on the center circle closest to the goal to be scored in. All players, other than the kicker, must be behind the midfield line and the player taking the kick. This is a "dead" kick. If a goal is not scored, the defense obtains possession with a goal kick.
Yellow and Red Card Policies: A player shall be cautioned (yellow card) for:
  • Entering or leaving the field of play except through the normal course of play without permission of the referee;
  • Persistent infringement of the rules;
  • Objecting by word or action to any decision given by the referee;
  • Any incidental use of vulgar language;
  • Unsportsmanlike conduct, such as, but not limited to, unnecessary delay (holding a shirt, short, etc.), deliberate verbal tactics, encroachment, deliberate hand ball to stop an attack, deliberate tactical foul, faking an injury; simulating a foul, etc.
A cautioned player shall leave the field and be replaced. An indirect kick is awarded to the non-offending team from the spot of the ball.
A player shall be disqualified (red card) for:
  • Taunting;
  • Subsequent caution;
  • Exhibiting violent conduct;
  • Deliberately handling a ball to prevent a goal;
  • Fouling an opponent who is moving toward his/her goal with an obvious chance to score;
  • Spitting at another person;
  • Using insulting, offensive, or abusive language or gesture.
A disqualified player shall leave the field and not be replaced. That player cannot return to play in that game. He/she shall also be disqualified from the team's next game. If the player was not on the field when given the red card the team may still play with three players but the disqualified player cannot enter that game or the team's next game. An indirect kick (or possibly a penalty kick) is awarded to the non-offending team at the spot of the ball.
Tournament Director: Will do everything possible to ensure that the official 3 on 3 soccer rules are applied to the games. She has final authority regarding all event disputes and issues.
Referees:
There will be one referee for up to two or more games. We are emphasizing good sportsmanship and integrity. If no referee is on your field at the time you will be required to make your own calls. Any argument is automatic Yellow card. Fights will an automatic dismissal from tournament.

At the end of each game the referee will report to the tournament director the result of the game. Teams are responsible, not the referees, or the tournament director, to know the time and place of their next game. Referee decisions are final. If there are any issues, please see the tournament director.
Ball Shaggers: Teams can provide their own ball shaggers or arrange themselves in a way to best keep balls from enter other fields.
Tournament Procedure: Each team will play at least three pool games. A seeded single elimination tournament will follow.
Scoring System to determine bracket winner:
  • 3 for a win
  • 1 for a tie
  • 0 for a loss
If a tie exists after pool play points have been calculated:
1. Head-to head results
2. Cumulative goal differential between goals scored and goals allowed
in all games played within the team's flight.
3. Least goals allowed in all games played within the team's bracket.
5. If tie still exists, the team with the most shut-outs will be determined the winner.
6. Finally, penalty kicks will determine the winner.
